We are looking for a compassionate, mission-driven Community Health Coordinator to help bridge gaps between patients and providers. If you're fluent in Dari and Pashto and want to serve refugee and immigrant communities through advocacy, care coordination, and outreach—this is your opportunity to do meaningful work. This is a temporary, grant-funded position.

Essential Duties and Responsibilities:

• Act as a trusted language interpreter between patients and care teams, ensuring patients fully understand care plans, medications, and follow-up instructions.
• Coordinate care by helping with referrals, connecting with pharmacies and hospitals, and preparing patients for procedures.
• Accompany and advocate for patients with complex medical or social needs during their appointments.
• Serve as a cultural navigator , organizing group visits, workshops, and community events to increase health access and understanding.
• Support outreach and partnership efforts aimed at strengthening care for immigrant and refugee populations.
• Accurately document interactions in patient medical records.
• Empower patients to build independence and confidence in navigating the healthcare system.

Required Education and/or Experience :

• At least 1 year of experience in community health, social services, or a healthcare setting.
• English language proficiency and comfort communicating in diverse environments.
• Fluency in Dari and Pashto is required.

Preferred Education and/or Experience:

• High School Diploma, GED, or equivalent.
• Certification in medical interpretation is a plus.

Other Important Details:

• This is a grant-funded , temporary position expected to last six months.
• Some travel required; must be comfortable using public transportation with patients.
• May occasionally lift items up to 25 lbs or assist patients in physical tasks.
• Work includes visits to off-site community locations and health partners.

Salary and Benefits:

• Compensation: The hourly rate for this position is $20 per hour.

Tapestry 360 Health is committed to equitable and transparent pay practices. In accordance with the Illinois Pay Transparency Act, we are disclosing the full salary range for this position. This range represents the potential compensation for the role based on experience, tenure, and performance over time.

Most new employees can expect an initial offer within the lower portion of the range, reflecting factors such as prior experience, internal equity, and organizational budget. Salary progression is evaluated regularly to support professional growth and retention.
